Through this notice, OPHTOS INSTITUT, S.L, hereinafter referred to as the OPHTOS INSTITUT, informs users of the website www.inof.es of its personal data protection policy, in compliance with Article 5 of Organic Law 15/1999, of 13 December, on the Protection of Personal Data, so that the user may decide expressly, freely and voluntarily, whether to provide the OPHTOS INSTITUT with any personal data requested, whether requested to provide information, or whether to book one of the ophthalmological medical services offered.
Correspondingly, users are informed that any personal data provided will be held on a file, duly registered at the Spanish General Registry of Data Protection, for which the owner and custodian is the OPHTOS INSTITUT (Corporate Tax ID: B63133227), with registered address at Calle Vilana 12, despacho 156-157, 08022 Barcelona, with said data processed with the aim of strengthening and maintaining the collaborative, contractual or business relationship established, as well as to receive advertising information about our services.
Unless specifically stated otherwise, it is necessary to offer all the data required on the form, or similar, in a true, accurate, complete and current manner. Otherwise, the OPHTOS INSTITUT may, depending on the case, decide not to proceed with the registration of the user, or may deny the specific service requested.
In the same way, and in accordance with what is established in the Spanish Data Protection Law, the user may exercise their rights of access, rectification, cancellation and opposition by sending a written communication to the OPHTOS INSTITUT, Calle Vilana 12, Despacho 156-157, 08022 Barcelona, with proof of his/her identity. The request must contain a name, surname and photocopy of the ID of the interested party or their legal representative, as well as a document proving legal representation, a document in which the request is made, an address for notifications, the date and signature of the applicant, and supporting documents for the request made. If the request does not meet the specified requirements, then any errors will need to be corrected. With regard to the right of access, this will only be denied when the request does not meet the aforementioned requirements and when no legitimate interest is shown to this effect. No remuneration will be required to exercise these rights.
Any requests regarding the exercise of the right of cancellation of identifying data for users who continue to maintain a relationship with the OPHTOS INSTITUT will not be met. However, users may object to the processing of their data with regard to receiving advertising information about our services.
The OPHTOS INSTITUT has taken the necessary technical and organisational measures to guarantee the security of this data, and to prevent its alteration, loss, processing or unauthorised access to the same, taking into account the technology used, the nature of the data stored and the risks to which they are exposed, whether from human actions or from physical means or the environment. All the technical and organisational measures required to achieve basic security objectives will be adopted, including: confidentiality, understood as the limiting access of unauthorised persons to the information; integrity, understood as maintaining the reliability and quality of the information; and availability, understood as guaranteeing access to the information system at the request of the authorised user.
In compliance with Law 34/2002 of 11 July on Information Society Services and Electronic Commerce, and the amendments introduced by Law 32/2003 of 3 November on General Telecommunications, it is noted that the acceptance of the present clause implies, in addition, the user’s consent to receive advertising communications for our services, as well as other communications that are not incompatible in nature. At any time, the user may object to their data being processed for these purposes, and may revoke their consent, by sending a written communication to the OPHTOS INSTITUT, Calle Vilana 12, Despacho 156-157, 08022 Barcelona, or by sending an email to agrafojo[at]inof.es, with the email subject title “DAR DE BAJA/UNSUBSCRIBE” .
